Quality of care is Christian Living Communities’ number one commitment to our residents, clients and families. In our rehab neighborhoods of Holly Creek and Someren Glen, their five-star rating reflects a nationally recognized measure of that commitment to quality. These communities have achieved the highest rating possible – 5 stars.

CLC is honored to receive this recognition, which affirms our commitment to an excellent, person-centered rehabilitation experience for our guests. We stand alone in our use of innovative, industry-proven tools such as PAM (patient activation measures), Interact (a care transition communication tool) and PCC (Point Click Care), which result in a higher rate of successful rehabilitation. Simply put, our guests’ transition back into home life is easier.

Holly Creek and Someren Glen earned rave reviews based on these results:

  • 5-star rating – Based on results from Medicare.gov.
  • 5 out of 5 Likelihood to Recommend to Others rating – Based on data from a Pinnacle Survey
  • Recipient of Skilled Nursing Best in Class honor – This Pinnacle award goes to the top 10% of the 800 anonymously surveyed former rehab guests.
  • Yearly Readmission Rate – Our rate for those being readmitted to the hospital after treatment is well below the State average fluctuating between 12-18% and National 18-20%, indicating a high rate of successful transitions back into home life.
