Starting January 6, 2025, the CLC communities Clermont Park, Holly Creek, and Someren Glen will have a new partner for in-community health clinics, Curana Health.
Curana Health specializes in providing comprehensive, on-site primary care tailored to senior living communities. Curana provides services in 38 states with a mission to improve health outcomes while ensuring that care is always convenient and accessible.
What Curana Health Offers
A wide range of on-site primary care services includes:
- Primary care visits
- Sick visits
- Lab work, X-ray coordination, and therapeutic injections
- Medication management
- Specialist coordination (e.g., cardiology, audiology)

- Supporting residents to age in place with dignity and comfort
Your Health, Your Way
Residents at all three communities may choose Curana Health as their primary care provider or rely on them for urgent concerns – it’s part of the Curana “Your Health, Your Choice” approach to care.
The dedicated team will work closely with residents’ current providers and specialists to ensure seamless, coordinated care that meets each person’s unique needs. The Curana team will follow its patients through the entire care continuum at each of the three communities.

Dr. Cody Gugenmos, DO
Dr. Cody Gugenmos, DO will provide physician oversight and care to older adults in the three CLC communities. Dr. Gugenmos received his doctorate in 2017 from Rocky Vista University right here in Colorado.
He is certified through the American Board of Family Medicine and part of the American Academy of Family Physicians. He has also worked with Hands for Health, an organization providing expeditions to offer global care. He grew up in nearby Nebraska, where he received his undergrad from Hastings University.
Joining Dr Gugenmos to provide in-clinic services are Nurse Practitioner Jessica Ponder and Physician Assistant Shannon Johnstad.
Jessica Ponder, FNP-BC, RN, MSN, BSN
Jessica received her Bachelor of Science in Nursing at Denver College of Nursing and obtained her Masters of Science as a Family Nurse Practitioner (FNP) from the University of Colorado-Colorado Springs.
Jessica has worked in primary and urgent care, but it was her experience in senior living that most touched her heart. She is looking forward to advocating for older adults. She will be in the clinics at Clermont Park on Tuesday and Thursday and at Someren Glen on Monday, Wednesday, and Friday from 9:00 am to 3:00 pm.
Shannon Johnstad, PA-C
Shannon earned her Master of Science in Physician Assistant (PA) Studies at Rocky Vista University and her Bachelor of Science in Biology from the University of Wisconsin-Superior.
Shannon quickly focused her PA practice on post-acute care and geriatrics working in long-term care communities. She has gained specialized experience in advance wound care. Shannon will be at the Holly Creek clinic Monday, Wednesday, and Friday from 9:00 am to 3:00 pm.

Each community also has a Medical Concierge in the clinics Monday through Friday from 9:00 am to 5:00 pm to help with care coordination and scheduling. 24/7 Triage Phone Support is available to all enrolled residents.
