Nursing mentors receive a Mentor Bonus of $300 – 500, paid over the course of a year in increments following completion of mentor and mentee evaluations. 

Through 98point6, CLC team members have access to low-cost, on-demand primary care delivered via secure text by board-certified physicians. 

Up to $6500 in resident-sponsored scholarship and tuition assistance funds available to further your education.

📢 Important Notice for Employees: Your 1095-C Forms are Available!

IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

We are pleased to inform you that your 1095-C forms, which provide important health coverage information required for your tax returns, are now available.

🔍 Where to Find Your 1095-C:

1. Online: You can view and download your 1095-C form directly from the company's employee portal. Simply log in to UKG at: https://n31.ultipro.com/Login.aspx?ReturnUrl=%2f

a. Username: First Name last digits of SSN (e.g. Robert 123-45-6789 would be Robert6789
b. Password: (default/1st time accessing) Birthday (MMDDYYYY)  (ex: Bob was born March 17, 1923 so is default password is:  03171923)
c. Once logged in, under the Myself icon in the upper left corner, scroll down to Benefits and select ‘Manage my Benefits’.
d. Under the new window, on the left-hand column expand Documents and select ‘ACA Documents’ section.

2. Request a Copy: If you prefer to receive a physical copy of your 1095-C form, you can submit a request through Christian Living Communities Benefits:

This link leads to the machine readable files that are made available in response to the federal Transparency in Coverage Rule and includes negotiated service rates and out-of-network allowed amounts between health plans and healthcare providers. The machine-readable files are formatted to allow researchers, regulators, and application developers to more easily access and analyze data.
